Grooming Client Application

Please fill out the information below to set up your client and pet profiles.
*Please Note* all dogs coming in for grooming only must be fully vaccinated with the following vaccines: Bordetella, Distemper/Parvo Combo, & Rabies. If your dog will be participating in Daycare or Boarding, they must also be vaccinated and current with a Canine Influenza vaccine. Daycare and Boarding requires all vaccines to be complete and up-to-date at least 7 days before expected arrival. This means that any boosters required after first time vaccination for canine influenza or bordetella must be complete as well. Initial vaccines without completed boosters do not satisfy our daycare and boarding vaccine requirements.

Grooming only dogs must be up-to-date on the required vaccines no later than 3 days before scheduled appointment.
